AML training (Must be completed)

If you do not have checks the carriers will accept the following:

                              Bank Letter

      Must be typed

      Must be on bank letterhead

      Account holder name must be in the letter

      Bank routing number & account number must be in letter

      Must be signed by a bank representative

                       Direct Deposit Form

      Must have a voided check on the form

      Must be pre-printed / Nothing handwritten

      Bank logo must be on the form

      Account holder info must be on the form

      Bank routing number & account number must be on the form
